Extracurricular Activities
WELCOME! Before you start filling out the application form, please note the following:
1. Your child can participate in a maximum of two sports. Please carefully double-check that the schedules do not overlap and fill out all the required fields of this form, avoiding typos/mistakes.
2. The First day of the Extracurricular Sports is Monday, August 14, 2022.
3. Please be informed that to sign up the siblings, you must fill out this form for each of them.
